In order to process your Evjue Foundation grant request we have designed an application form we hope you will find easy to follow and complete. (Click here to go to the Dropbox site for the form and then click on the blue "Download" button in the upper right hand corner.)

Applications can be submitted via mail, email or in person. Mail the completed form to: The Evjue Foundation Inc., 1901 Fish Hatchery Road, Madison WI 53713 or email to pwells@madison.com.

Remember: The application must be limited to two pages. Do not alter the form in any way and please do not send any attachments other than a copy of your IRS tax exempt status letter. All details regarding your grant request must be included on the form itself. If further clarification is necessary, we will contact you.

Our fiscal year runs from March 1 to February 28 and as a general rule, the Foundation accepts one grant request per fiscal year per organization. We accept applications at any time, and will hold them until the next meeting of the Board.

We have a spring and a fall meeting each year to consider grant requests. Our next meeting is scheduled for November 2017. Applications must be received by 5 p.m. on Tuesday, Sept. 5, 2017. If you have any questions, please call me at 252-6282 and I will be happy to assist you.
